Use your Humana VCP benefits for routine eye exams.
Humana VCP, or Vision Care Plan, is a vision health coverage plan. The plan provides discounts on benefits such as a pair of eyeglasses, contact lenses, PRK and LASIK services. You pay a co-payment for an annual routine eye health examination. You have the option of using an in or out-of-network provider; however, you qualify for a higher savings when you use a provider within the network. Once you become a VCP member, use your benefits for maintaining vision health.
Instructions
1. Receive your Vision Care Plan ID card in the mail after signing up for the plan. If you don't receive your ID card, contact customer care at (866) 537-0229.
2. Visit the Humana Vision Plan website and select a network provider prior to scheduling your appointment. Click "Locate a Vision Care Doctor," enter your address or ZIP code in the boxes provided and find a provider in your area. Look on the back of your ID card and locate the number for the Humana VCP customer care department. Tell the representative which provider you prefer.
3. Call the provider's office and schedule an appointment. Provide your name, contact information, member number and any other information requested for your appointment set-up.
4. Arrive at the provider's office at the time of your appointment and present your Humana VCP card. Sign the provider's VCP form after completing your examination. Pay any co-payments or other charges not covered by the plan.
