CHIP is the Texas health program for children.
CHIP is a Texas health insurance program available to resident children. The coverage is available to children whose care providers earn too much money to qualify for state Medicaid, yet do not earn enough to pay for private insurance. CHIP recipients pay low enrollment and co-pay fees, and covered dependents receive free well checks and vaccinations.
Uninsured pregnant mothers can also apply for CHIP. Perinatal coverage provides benefits for pregnancy related check ups, labor and delivery. In addition to these services, two post natal check ups for the mother and the first 12 months of infant check ups and vaccinations are covered.
Instructions
1. Determine if you have a qualified dependent. Generally, a qualified dependent is an uninsured child under the age of 18. A parent, stepparent, grandparent, legal guardian or other adult relative may apply for coverage if the child lives with them.
2. Determine if you meet income restrictions. Households with two members can earn $2,429 per month and receive benefits, while households with three members can earn $3,052 and a household of four can earn $3,675. See the link in References below for additional family size guidelines.
Families that earn significantly less than these maximums may qualify for Texas medicaid instead of CHIP.
3. Fill out an application online, or request that a paper application to be mailed to you.
4. Be prepared to provide proof of income and expenses. Mothers applying for perinatal coverage should submit Form 3037 to provide proof of pregnancy.
